Complete the application form below to have your ADULT Deed Poll for Change Of Name drawn up by our legal team.

You will need to name a Witness in your application. We need the Witness's name, address, and occupation and they must be at least 18 years of age.

When you receive the Deed Poll in the post you must sign it with your Witness in the presence of Commissioner for Oaths or a Solicitor, who must also co-sign and then submit to the Register of Deeds at the High Court.

Even with a Deed Poll your name on your Birth Certificate cannot be changed. When  asked to present your Birth Certificate you also present your Deed Poll as proof of your entitlement to use your new name

Do note when changing national or travel ID to your new name , you may need to prove use of your new name for a period before you will be allowed to use your new name on your ID.
